If you are in need of academic support because of a documented disability (whether it be psychiatric, learning, mobility, health-related, or sensory) you may be eligible for academic accommodations through Disability Services. As a student with a disability, you have certain rights that are guaranteed to you by law. Section 504 of the Rehabilitation Act of 1973 states that, "no otherwise qualified (disabled) individual shall, solely by reason of their (disability), be excluded from participation in, be denied the benefits of, or be subject to discrimination under any program or activity receiving federal financial assistance."

Requesting Accommodations

Required Forms to Complete:
*High School 504 and IEP plans can be submitted, however, High School plans and documentation will only be used for supplemental information when creating a College 504 plan.

To submit the Request for Accommodations and the Disability Documentation forms:
  • Email the documents to the Accessibility and Student Success Manager (contact info below); or
  • Hand deliver them to the Accessibility and Accommodations Office in Student Services (Success Center) at the Ontario Campus or Caldwell Center; or
  • Hand deliver them to an Enrollment Specialist at Student Services at the Ontario Campus or Caldwell Center; or
  • Mail the documents to:

  • Your Documents will be assessed by the Accessibility and Student Success Manager. 
  • You will be contacted to set up an appointment to review your accommodation requests.
  • A College Student Accommodation plan will be created and appropriate accommodations will be afforded to you.
  • An Accommodation Letter will be available to you to present to your instructors.
